Establishes the Affordable Medicaid Buy-In Program by January 1, 2027, allowing West Virginia residents ineligible for Medicaid or Medicare to purchase state-administered health coverage leveraging the Medicaid structure

Covers 10 essential health benefit categories including hospitalization, mental health and substance use services, prescription drugs, maternity care, and preventive/wellness services

Requires a sliding-scale premium structure with discounted premiums and cost-sharing for individuals with household incomes below 200% of the federal poverty level, with full-cost purchase available for those who don't qualify for assistance

Mandates the Department of Human Services apply for federal Basic Health Plan (Section 1331) and/or State Innovation Waiver (Section 1332) options to maximize federal funding

Creates an advisory council including the Secretary of Human Services, Insurance Commissioner, consumer advocates, health care providers, and public health experts to guide implementation and evaluate the program
