Creates the "Insurance Medicaid Buy-in Program" allowing West Virginia residents ineligible for Medicaid or Medicare to purchase state-administered health coverage leveraging the Medicaid structure, with implementation required by January 1, 2027

Requires comprehensive coverage including hospitalization, emergency services, mental health and substance use treatment, prescription drugs, maternity care, preventive services, and pediatric care including dental and vision

Establishes a sliding scale premium structure with discounted rates for households earning below 200% of the federal poverty level, while offering full-cost purchase options for those who don't qualify for financial assistance

Directs the Department of Human Services to apply for federal Basic Health Plan Option (Section 1331) and/or State Innovation Waiver (Section 1332) under the Affordable Care Act to maximize federal funding

Prohibits employers from disenrolling or denying enrollment in employer-sponsored coverage based on an employee's potential eligibility for the buy-in plan
