Requires the Department of Health, Department of Human Services, Department of Health Facilities, and Office of the Inspector General to prepare annual performance plans covering each program activity in their budgets

Performance plans must establish quantifiable performance goals, indicators, and outcome measures that compare results to state and national rates

Departments must submit annual performance reports to the Legislative Oversight Commission beginning July 1, 2026, with progress reports on baseline data due July 1, 2025

Reports must include multi-year comparisons of actual results versus goals, explanations for unmet goals, and corrective action plans for underperforming programs

Departments must report within 30 days any incident of serious harm or death involving individuals in state care, custody, or who had a referral within the past six months
