Requires West Virginia Bureau for Medical Services to implement Medicaid work requirements by January 1, 2027, in accordance with the federal One Big Beautiful Bill Act (P.L. No. 119-21)

Prohibits undocumented immigrants from receiving Medicaid benefits effective October 1, 2026, and bans multi-state enrollment of beneficiaries by January 1, 2027

Mandates eligibility verification at least twice annually and quarterly removal of deceased enrollees from Medicaid rolls, both effective January 1, 2027

Limits retroactive eligibility to 30 days for expansion enrollees, 60 days for traditional Medicaid, and 30 days for CHIP enrollees by January 1, 2028, while requiring cost sharing for expansion adults by October 1, 2028

Requires the Medicaid error rate to be reduced to 3% or below by January 1, 2028, and mandates all Medicaid waivers be budget neutral
