Creates the Wyoming Rural Health Transformation Program within the Department of Health, funded by federal grants under section 71401 of the One Big Beautiful Bill Act (P.L. 119-21), with 80% of first-year funds and 69.5% thereafter deposited into a new perpetuity fund

Establishes a 9-member advisory committee appointed by the governor to oversee fund operations and approve grants/expenditures exceeding $500,000

Allocates perpetuity fund annual distributions to: critical access hospital incentives (40.7%), EMS regionalization (26.9%), workforce education individual support (21.6%), and physician graduate medical education support (10.8%)

Authorizes time-limited initiatives including integrated primary care expansion (36.4%), technology adoption grants (15.1%), exercise/diet programs (9.8%), tele-specialist platform (8.2%), workforce education startup costs (8.2%), and care coordination grants (8.2%)

Requires recipients to enter performance and accountability agreements with operating margin targets, liquidity benchmarks, and efficiency requirements, with repayment provisions for noncompliance or misuse of funds
